How to Register

In Person

If you would like to register as a patient at the Practice, please check first that your address falls within our main practice boundary area.  You can do this at the surgery or by using our boundary map and postcode checker above.

Please either call in and collect a registration pack from reception, or print off the registration documents from the links below to complete at home.  Please note that an NHS registration form is required for EACH adult or child wishing to register.

You may like to print off/view a copy of our Practice Leaflet, which contains further information about the surgery and our services. You may also wish to print/view a copy of our Patient Charter which explains a little more about our commitments to our patients and how we work.

Please complete the NHS registration form and our new patient questionnaire, answering all the questions fully.  Failure to do so may cause a delay in completing your registration.

Once completed, please bring your forms and the appropriate ID documents in to reception.  You will need to sign the forms at the surgery.  Your forms will be checked and you will be notified of your Named & Accountable GP.

Alternatively you may prefer to print off a registration form and medical questionnaire form, fill them out and bring them in with you on your first visit to the practice.  Please click on the Registration Page to print these forms.

We also politely request that you bring in a form of ID if at all possible or take a photograph holding your ID and email this to bnssg.priory.surgery@nhs.net.
