Referrals During the Covid-19 Pandemic

There have been reports in the media stating that NHS routine referrals are back up and running.

At a local level, these are being restarted at different times and it is still the case that some routine referral pathways remain closed.

We are monitoring the situation regularly. If a GP provides you with a routine NHS referral, our referrals team will only send this when it is possible and will contact you once this has been processed – this may be via text message.

May we therefore politely ask that you do not call the surgery to chase your referral. The situation is changing on an almost daily basis so please continue to bear with us through all this.

The above does not apply to suspected cancer or urgent referrals.

Key Information – COVID 19 and Domestic Abuse

The household isolation instruction as a result of coronavirus does not apply if you need to leave your home to escape domestic abuse. Even during lockdown, if you are in danger in your home, please leave and seek help. Call 999 in an emergency.

There is now a freephone, 24 hour National Domestic Abuse Helpline number

New Patient Registration

If you would like to register as a patient at the Practice, please check first that your address falls within our main practice boundary area.  You can do this at the surgery or by using our boundary map and postcode checker.

Please either call in and collect a registration pack from reception, or print off the registration documents from the links below to complete at home.  Please note that an NHS registration form is required for EACH adult or child wishing to register.

Our most recent Practice Newsletter is available by clicking the link on the right of the screen under "Latest News".

Please complete the NHS registration form and our new patient questionnaire, answering all the questions fully.  Failure to do so may cause a delay in completing your registration.

Once completed, please bring your forms and the appropriate ID documents in to reception.  You will need to sign the forms at the surgery.  Your forms will be checked and you will be notified of your Named & Accountable GP.
